זהו הלהיט הראשון של Blue Öyster Cult, להקת "פולחן הצדפה הכחולה". כתב אותו הגיטריסט המוביל דונלד רוסר Donald Roeser שהיה מוכר גם בשם Buck Dharma. וא גם מי ששר את השיר במקור. היו שמועות שהשיר מתייחס להתאבדות, אבל כותבו כיש מכל וכל: "נדהמתי כשקראתי שאנשים רואים בשיר מעין עידוד להתאבדות". במשמעות הפשוטה והישירה שלו הכוונה היתה שיר אהבה, המטיף לאמונה, שאנחנו לא צריכים לפחוד מהמוות ושהאהבה תנצח אותו. כשרוסר/ דארמה כתב את השיר, הוא חשב על מוות בגיל צעיר והאם הוא יפגוש באהובתו בחיים שלאחר המוות.
Blue Öyster Cult נחשבה ללהקת "קאלט" – מוסיקה בהשפעות מטאליות של להקות כמו Black Sabbath וטקסטים מוזרים העוסקים בעל טבעי. "Don't Fear The Reaper" היה דווקא שיר פחות מתוסבך שחשף את הלהקה לקהל גדול. הגם שכלכלית, השיר שיפר את מצב הלהקה, אומנותית – חבריה לא נפלו על השיר. הם מעידים כי מאז השיר הזה הופעל עליהם לחץ לכתוב עוד להיטים, מה שלא עלה בקנה אחד עם האוריינטציה האומנותית של הלהקה.
חברי הלהקה בעת הקמתה ב-1969 היו אריק בלום (גיטרה, שירה), ג'ו בושארד (בס), אלברט בושארד (תופים), אלן לאנייר (קלידים, סינתיסייזר), דונלד"באק דארמה" רוסר – גיטרה מובילה) כ-15 נגנים התחלפו בהרכב הלהקה, שהוציאה את אלבומה האחרון ב-2001, אבל המשיכה להופיע עם שני חבריה המקוריים באק דארמה ואריק בלום
